This center really made me feel comfortable leaving my family member in. The nurses are caring, compassionate, and most importantly, competent. Staff looks happy which makes me feel as if management treats then appropriately. Of course no one ever wants to have to use a nursing home for their family but I'm so glad I was able to find one that I feel is truly good. Definitely recommend!

Being in a nursing home is an uncharted experience for most families, but at Port Orange it has been a great transition for my loved one and our family. Robert in Social Services and Caitlyn in activities has been a big part of making this easier. The administrator Ashley is very responsive and involved in all aspects of care.

The pt staff are great! Some of the nurses are wonderful. Food sub-par.
Something suspicious goes on after regular hours involving staff and others.
Needs to be investigated!!
I would warn anyone not to

I can honestly say this facility is one of the best facilities in the area. They have a proactive approach to your loved one's care. Upon entering the building, you will be greeted by Christina, the receptionist where she will give each visitor in your party a badge. I know sometimes there may be a line and you might have to wait, but that is only to protect you and your love one, in the event of an emergency, the facility has an accurate count of staff, visitors, and residents. The doors stay locked. You can press a buzzer to enter, collect a badge, and visit your loved one. Keep that badge because you will need it to get out. Visiting after hours, no problem. They have an additional buzzer, that goes directly to the floor. You might have to wait a minute because the staff member has to walk down to the front to let you in. When your loved one arrives for admission, they will be greeted by a nurse and a CNA. Be patient. It may take some time. This facility is busy. On average they are getting two to three admissions a day. A nurse can have anywhere from 1 to 40 patients while a CNA can have anywhere from 1 to 20. I can assure you this facility staff's well above the STATE guidelines. The CNA may have anywhere from 1 to 15 on a bad day and the nurse may have anywhere from 1 to 30. You may hear a staff member from time to time. Say we are working short today, That's not true. They are just spoiled because usually they have anywhere from 1 to 10 patients.
Your loved one will get 3+ meals a day. There is a selective menu that they can fill out a week in advance. If their meal tray arrives in the room and they don't like it, they can always ask for an alternative which usually includes grilled cheese, chicken tenders, a hamburger, or many other options. Tony, the dietary manager will come around and get your loved ones likes and dislikes. And while we still have to follow your loved one's diet, Tony will certainly try to include their likes. I've seen Tony go as far as making special meals for patients who are refusing to eat or losing weight.
Your loved one does not have to just lay there in the bed like a hospital. They do not have to just stay in their room. There is a wonderful activities department that is ran by Caitlin , Shannon, and Shirley. Everyday these ladies have some type of fun entertainment like live music, bingo, popcorn in a movie, coloring, arts and crafts and much more.

This facility has one of the best therapy departments I've seen. They have physical therapy, occupational therapy and speech therapy. All three departments. Have a wonderful therapy manager named Stacey. Stacy has been with the company for over 20 years. If you like the therapy, your loved one is receiving in the facility, once they are discharged you can bring them back for outpatient therapy.

Looking to arrange a discharge? You can talk with Bob or Candice from social services. They usually set everything up for you. Whatever equipment you need upon discharge, to your next doctor's appointment. Want your loved one to stay long-term care or transfer to another facility. They can help with that too. Missing an item or have a complaint. Have a staff member help you fill out a grievance. They usually find and return the item quickly. Or as long as it's on your loved one's inventory sheet, the facility will cover it.
Trayshawn in the business office is good at notifying you of your loved ones co-pays or insurance updates. She can also help with the Medicaid application process.
The management team here do a phenomenal job. They ensure that your loved one has the best care possible and that all policy and procedures are being followed. However, they are not mind readers. They do not know something is going on unless you speak up and tell them. I can assure you they are quick to resolve whatever issues you or your loved one is having.
My time has come to an end at this facility, by my own choice, but I can assure you, your loved one is in good hands!

The nurses and staff are attentive and treat loved ones like family. Floretta, and Maureen, always smile and are as friendly as it gets, When you walk in there is always a warm smile from Nicole.and usually Maureen is there with an enthusiastic hello. Nurses Beth, Cheryl, Nyree, Rodney, Laurie and other are all great and the therapy staff is great. If we ever had an issue Justin and his team were quick to listen and implement change.

During the time my wife was a patient there for 180 days overall the care was very good to excellent depending on the treatment provider. Considering the length of stay there will be both good and not so good experiences but neither should solely determine the final result of returning home and feeling better. It seemed that the CNA’s who carried a lot of the workload had to many patients and not a lot of time to check on them but once or twice during a shift. However, they did fine. Tho PT/OT staff was outstanding and deserve much credit for their commitment and professionalism in restoring her functionality. Over this lengthy stay we met and interacted with so many wonderful and caring staff that to try and name all and miss one would be a disservice to the omitted and an embarrassment for us. So to every member of PONR who touched our lives in such a meaningful way, Thank you! Liz&Tony Ziner

Kudos to Shirley Stokes Hill, CNA. Shirley is a very sweet lady who is right there when you need her help. She is fast as lightning in getting her job done. She has an excellent personality and tries to help everyone she can. She is a stellar employee and a true asset to the Port orange Rehab Center. Thank you, Shirley for all you've done for me in the four weeks that I was in rehab.

I work closely with Candace a case manager to help patients with durable medical equipment and she is always a pleasure to work with. She works very hard to obtain the documentation needed in order to bill the patient's insurance so out of pocket cost is low to none.
